Overall, this is the best example of how poor management turns a hospital into almost a prison.

I was in the 9th department as a military man in the summer and fall of 2017. In the department, all together, there are servicemen of the Russian army (all of them, without exception, are absolutely healthy, they are simply dismissed from the army), conscripts sent for examination from the military registration and enlistment office (most often they are also dismissed from the army) and truly mentally ill people (they are usually called civilian). In terms of the ratio of military and military registration and enlistment officers together, there are slightly more than civilians.

Local restrictions. Phone, cigarettes, lighters, matches, watches, jewelry (rings, chains, etc.), documents and outerwear are handed over upon admission and returned upon discharge or weekend leave. Upon receipt, the soap and snout is packed into a bag and sent to the so-called old staff room. Then, every day in the morning when you get up and in the evening shortly before lights out, the old staff room is opened and you can take soap and snout (you cannot store soap and snout in the ward). The shaving accessories go with the soap and soap, but you can take them only if the orderly gives permission. When will the orderly give permission? When he wants it. Phones are officially issued on Thursdays and Sundays for 20-90 minutes, depending on the nurse’s mood. Unofficially, military personnel are issued phones for the evening or overnight in about two out of three shifts of nurses. Every morning (or rather, before each shift), the orderly walks through the wards and checks the contents of the bedside tables and can also look under the bed and under the mattress; accordingly, everything prohibited goes either into personal belongings, or, if it’s cigarettes or a lighter, into a pocket to the orderly. You cannot leave the department. They take him out for walks in a small pen either once every 3 days or 2 times a day, depending on the mood of the orderly and nurse. Military personnel and military registration and enlistment officers are taken out a little more often, since they can be taken out by a nurse from a military hospital, who comes 1-3 times a week. Upon arrival, civilians are dressed in local pajamas, which are already worn out to such an extent that they are already see-through. Upon admission, military personnel are dressed in decommissioned hospital trousers (as in a military hospital), but are not given a hospital shirt. Only military registration and enlistment officers and civilians who have been removed from supervision and who have something to wear can wear their own clothes. Smoking is not allowed here, but all smokers smoke quietly in the toilet. Cigarettes are prohibited here, but military personnel have the opportunity to go to the store and buy them. It is also possible to discreetly pass cigarettes on a date. Visits are held once a day at certain hours and with a pass from the doctor and last as long as the orderly or nurse allows (no more than an hour as a rule). 6-9 people live per ward (the norm is no more than 6), but there are also 4-bed wards (the norm is no more than 2). Shower on Tuesday mornings, then you can get clean clothes, and no one cares about your wishes. For the military, concessions are made and they are allowed to shower every day in the evening. By morning, food should either be eaten or put in the refrigerator. The refrigerator is opened twice a day or according to the nurse's mood. And also, as a cherry on the cake, the toilet closes after plus or minus thirty minutes after getting up and opens after plus or minus an hour and a half. It also closes once a week at an unpredictable time for an indefinite period of time (depending on the mood of the orderly) for general cleaning.

Local workers. You can say about any orderly and nurse that this is a person with a mood. The orderlies are incredibly ignorant (typical graduates of village schools), but at the same time they are very kind people and can often get the hang of things. Nurses are no less ignorant, but they get into the situation much less often. Doctors don’t care about patients at all; you can be detained for as long as you want until you or your parents start pestering the doctor about your discharge. There is also a sister-hostess, she can get into the situation, she is ready to help all the military men who help her. In general, everyone is afraid of being reprimanded by their bosses; if the bosses leave, everyone will immediately become kinder.

Local regulations for civilian and military registration and enlistment officers. At 7 am I get up, brush my teeth and wait for breakfast. Breakfast at 8 am. Around 1pm lunch. Afternoon tea at 4 p.m. From 16 to 18 hours of appointment (on weekends also from 10 to 12). Dinner at 17:30. At 21:30 brush your teeth and get ready for bed. Lights out at 22:00. When eating, they sit down after the military sit down. If you were able to bring in money, you can buy cigarettes from the military at a premium.

Local regulations for the military. At 7 a.m. we get up and need to take the empty cans to the catering unit (a neighboring building). At 7:10 on weekdays, someone must take the bulletin to the next building and someone must take out a bag of trash. At 7:30 someone goes to the catering unit for breakfast. When breakfast is brought, someone (yes, the military does all this) must carry the food to the tables. Three times a week at 9 o'clock someone needs to take bags of dirty bed linen to the next building. At 10:45 from Monday to Thursday you can go to art therapy (draw, chat and sit quietly on your phone if you were able to hide it). At 11 am, someone needs to go with the barmaid to get compote. At 12 o'clock someone must go to the catering department with the barmaid to buy bread. At 12:30 a few people go with the barmaid for lunch. When dinner is brought, someone needs to serve it, but the military only does this on weekends (after all, the authorities see what you mean). Three times a week at 2 p.m. you need to bring bags of clean laundry from one neighboring building. Also, at 2 p.m. you may be scheduled for group psychotherapy, which runs from Monday to Friday. At 15:30 you need to go with the barmaid for afternoon tea. You might be able to pour yourself some tea and grab some cookies right away if you ask. The rest have lunch at 16:00. At 17 pm you need to go for dinner. And when dinner arrives, someone needs to serve it. At 17:00, someone must also take the report to another building (in total there are 6 different neighboring buildings and 1 garbage dump). At 6pm it's time for someone to take out the trash. After dinner, they will probably give out telephone numbers, if an agreement can be reached. Closer to lights out or after lights out (depending on the medical system) you can ask for a shower, which you will wash yourself afterwards. These things are usually negotiated by the sergeant major (this is not a rank, this is an unofficial position of one of the privates). After lights out, if there is no male orderly on duty at night, some two soldiers will need to start being on duty at the post (this is a table and two chairs about between 6 and 8 wards). At approximately 2 a.m., the duty personnel change, the second shift is on duty until rise. As a rule, the foreman appoints the duty officers, chooses who will go for food and who will take out the trash. Also, from time to time, military personnel can go to help (help = carry something or someone) to other departments or to a warehouse. Other branches usually pay with a pack of cigarettes. Assistance, as a rule, includes carrying a heavy patient on a stretcher, tying up and accompanying a violent patient to the department from the reception, and transferring boxes from the car to the warehouse.

Local population. The majority are reasonable people, but there are some who are simply fools and those who cannot be described in decent words. No surprises here. Of course, due to the specifics of the establishment, there are funny characters here, but over time you get used to them and don’t react. We meet literally everyone: from graduate students to prisoners, from military personnel to those serving time in the hospital due to a serious illness (schizophrenia). Here they eat garbage and sometimes howl at night. However, there are no fights, no one rushes.

Local food. She's just terrible. Anyone who was in the army will be surprised that it can be even worse (most likely). In the mornings they give porridge very diluted with water, tea without sugar and yesterday’s bread. For lunch it will be super, sometimes there is good borscht, but usually soup “with cabbage, but not red”, a second with a small piece of chicken or fibers (yes) of chicken meat and a tasteless side dish, compote (sometimes jelly) without sugar and bread, which was not eaten at breakfast (seriously). For afternoon tea, tea without sugar and either two cookies or a casserole, sometimes even sweet. For dinner, some kind of porridge and either fish (pollock is the worst), or an egg, or an omelette, or a slice of doctor's sausage, and unsweetened tea with, finally, today's bread, as usual. At 21:00 they sometimes give you unsweetened compote and bread. At any time of the day, if you are a military man, you can ask for bread and most likely you will receive it. It is advisable to always have your own sugar and spices in order to somehow improve the taste of the food; they are allowed to be stored in the room. You can ask a nurse for boiling water and it is quite likely that you will be refused, but not always. By the way, you cannot keep tea and coffee in the room.

Local smoking culture. Actually smoking on site medical institutions prohibited by law. So either on the street, or, when the bosses leave (after 5 pm somewhere), in the toilet. Cigarettes are obtained in two ways: military personnel occasionally run to the store and buy, and relatives bring them and discreetly hand them over on a date. The orderlies and nurses will take away your cigarettes and matches/lighters as soon as they see you. However, everyone, including managers, is well aware that people smoke in the toilet and on the street. The heads of some departments, knowing this, do not prohibit anything, but not our head. Why is the head of the 9th department complicating everything? Unknown.
