Filling out an application for a Schengen visa sample. How to fill out an application for a Schengen visa on your own - or maybe it’s better to turn to a “pro”? Filling online: subtleties and nuances

Upon receipt of a Schengen visa by one of important documents is a visa application. Its correct filling is the key to further success in obtaining a permit document.

Visa Code of the Schengen Agreement and the role of the visa application form

The Visa Code of the Schengen Agreement, which is valid when applying for a Schengen visa to any of the states that have signed this interstate agreement, provides for a single visa application form. It contains a standard set of questions. Questionnaires have some differences depending on the country to which a visa is requested. But they make up no more than five or ten percent and are primarily in the language in which the items of the questionnaire are duplicated. There are various designations of countries on the forms, as well as contact information in a footnote. The remaining 90% of the content of this document is universal for all states.. The filling rules are also the same for all 26 Schengen countries and practically do not depend on the country to which a visa is requested.

It is the questionnaire that is the first document that must be filled out when obtaining the coveted entry permit to the Schengen countries. Its correct filling in many ways contributes to making a positive decision on issuing a visa, and also prevents delays in the procedure itself.

The Schengen Agreement includes 26 countries

Who issues the visa application

The immigration law of the European Union requires that initially a Schengen visa be issued personally by the applicant. In the future, the questionnaire can be filled in person or through the visa center, as well as through a travel agency. In this case, the employees of the company issue a questionnaire, where you need to enter all the information. Then they themselves fill out a visa application form, taking into account all the rules for entering data. But the personal signature of the applicant must be in any case.

Since 2015, there has been a requirement to submit biometric data, which involves taking fingerprints for inclusion in a single immigration database of the Schengen countries and the European Union. Despite the fact that far from all EU states are included in the Schengen Agreement, they have full access to the immigration database. Information about biometrics will be stored for 59 months, during which there is no need to re-take fingerprints when applying for a second visa or its extension. It is enough just to indicate the date and place of the previous fingerprinting.

The need for fingerprinting appeared in 2015

In some cases, it is allowed to write a questionnaire and submit documents through a representative by proxy. But this applies only to those who cannot do it themselves, for example, for health reasons (a medical certificate will be required). Questionnaires for minors who are under 16 years of age are filled in by their parents and signed by them. Teenagers over 16 must complete and sign on their own.

Is it difficult to fill out a Schengen visa application form on your own? The most difficult thing to cope with this task is for those who apply for a visa for the first time. Perhaps in this case it is worth consulting with those friends, relatives or just acquaintances who have already visited Europe and can help in filling out this document correctly. You can contact the visa center or travel agency. In this case, the questionnaire will certainly be filled out correctly. But for services sometimes take a lot of money. The question is, is it worth overpaying if you can handle this task yourself? In addition, the next time you receive a visa, filling out the questionnaire will not present any difficulties for an applicant who already has such experience. Detailed instructions for filling

As already mentioned, each of the points of the visa application form must be completed. And not just filled out, you need to do it right so that visa officers do not reject it during the interview and check documents and do not have to go through the whole procedure again from the very beginning. So:

  1. Paragraphs 1, 2, 3. They must indicate the current surname, maiden name, as well as all the surnames that the applicant or applicant has ever had. You should also write a name. The spelling must fully comply with the transliteration that is in the foreign passport.
  2. P. 4. The date of birth is written here. The year must be entered first, followed by the month and day.
  3. Clause 5. This is information about the specific city and region of the applicant's birth.
  4. P. 6. In this paragraph, you must write the country of your birth. The state in which the visa applicant was born is indicated. For example, he was born in 1970 in St. Petersburg. Most applicants applying for Schengen visas for the first time record their place of birth in this way: Russia and St. Petersburg. But this is absolutely wrong. It is necessary to write the name of the country, which included the current St. Petersburg. The country was called the Union of Soviet Socialist Republics, abbreviated as USSR or USSR. And the city of St. Petersburg in those days was called Leningrad. Therefore, in the columns "country and place of birth" should be written like this: the USSR, the city of Leningrad. The same requirements are relevant for everyone who was born in the Soviet Union before its collapse (1991). The name of the city must be the same as at the time of the applicant's birth. You can write the modern name in brackets.
  5. Clause 7. Here it is necessary to indicate all the citizenships that the applicant has at the time of filling out the visa application form, as well as those that he had throughout his life.
  6. P. 8. You will need to indicate the gender of the applicant.
  7. P. 9. This paragraph indicates information about marital status. In this case, it is necessary to be guided by the marks that the registry office put. When filling out the item by an unmarried girl / unmarried young man, “single” or “single” is written.
  8. Clause 10. This includes information relating only to minor applicants for a Schengen visa. All persons with parental or guardianship rights will need to be listed. When the parents of a minor visa applicant are divorced, they still remain the parents of the visa applicant, unless there is judgment to terminate their parental rights.

At the beginning of the questionnaire, the personal data of the applicant are indicated.

  9. Item 11. This will require information about the national identification number. Citizens of Russia do not have it. This paragraph may be relevant for applicants who have a residence permit in countries where such a number is assigned.
  10. Clause 12. Here we are talking about the category of document for foreign trips, which the applicant possesses. A foreign passport can be a service, diplomatic or simple foreign passport. In the event that the visa applicant has several international passports, you will need to indicate the one on which he plans to cross the borders of the Schengen area.
  11. P. P. 13, 14, 15, 16. In these paragraphs, you must specify all the information about the passport: when and by whom it was issued, how long it is valid.
  12. Item 17. In this paragraph, you will need to indicate your home address. It is necessary to write the one that is the actual place of residence. Also in this paragraph, information about the applicant's e-mail address and contact phone numbers by which he can be contacted is required.
  13. Item 18. This item can only be completed by applicants applying for a Schengen entry permit in a country other than their country of citizenship. For example, a citizen of the Republic of Belarus permanently residing in Russian Federation, is such a person.
  14. P. P. 19, 20. Officially employed applicants for a Schengen visa in this paragraph write information about their place of employment, contacts of the employer and the nature of their activities. Students will be required to indicate the name of the specialty in which they are studying, as well as all contact information for the administration of the higher education institution. The unemployed simply mark their status.

As a contact phone number, you need to indicate the current number, because the consulate can call to clarify the data, and it is undesirable to miss such a call

  15. Item 21. Here you should indicate the purpose of the trip to the Schengen countries. The one that is most like is chosen, or your own version is entered by hand. It is important to remember that the goal must be supported by documents. For example, if this is a guest visit, then an invitation is required and the name and contact details of the inviting person must be indicated in the questionnaire. If the invitation is issued by an organization, then its name and detailed details will be required;
  16. P. P. 22, 23. The country of primary residence is indicated. This is the name of the state in which the Schengen visa applicant plans to stay the longest. The following information refers to the country whose border will be crossed first when entering the Schengen area.
  17. Item 24. Here it is necessary to indicate the required number of crossings of the Schengen border. This refers to the category of the requested visa. It can be single, double or multiple (multishengen).
  18. Item 25. You will need to indicate the planned duration of the trip. For single entry visa This is the number of days from the date of entry into the Schengen area to the date of departure. For multi-schengen, 90 is always set.
  19. Item 26. Information on previously issued Schengen visas to any country is entered here. Data about them is copied from the passport.
  20. Item 27. You should indicate the date when the biometric data submission procedure took place (if known). If it was not carried out, the “No” item is selected, and then when submitting documents, the applicant will be fingerprinted.
  21. Clause 28 is provided for transit passengers (when a transit Schengen visa is requested when traveling to another country). Enter the data on the visa to the state of main destination in the paragraph.
  22. P. P. 29, 30. These are the validity periods of the requested visa. For a single entry visa, the expected dates of crossing the borders of the Schengen area for entry and exit are indicated. For multischengen it is general term visa validity (six months, a year, etc.).
  23. P. P. 31, 32. If travel is made by invitation, then in these paragraphs it is necessary to enter information about the persons or organizations that issued it. If a tourist visa is requested, the address of the hotel where the rooms are booked is indicated. When the Schengen visa applicant does not know the exact address, you can simply indicate the name of the city, as well as the type of accommodation.

It is desirable to indicate the contact details of the receiving party in as much detail as possible.

  24. P. 33. This is a very important point regarding the material support of the trip. Here you should specify who exactly will bear the costs of paying for the trip. You can tick several options at once. If there are enough own funds, then the applicant indicates that it is he who pays for his trip. If we are talking about a sponsor, then you will need to indicate his last name, first name, patronymic, as well as contact details. It is necessary to specify this information in more detail and note the payment for transport, housing, food, and so on (who exactly will pay for what). It is also necessary to enter information in what particular ways the applicant is going to pay: in cash, bank cards, and so on.
  25. P. P. 34, 35. In these columns you will need to enter information about relatives living in the European Union or Switzerland. If the applicant has any, then it is necessary to indicate their names and surnames, place of residence and contact details, as well as inform the European immigration authorities about the degree of family ties.
  26. P. P. 36, 37. In the last two paragraphs of the questionnaire for obtaining a "Schengen" information is entered on the name of the city in which the visa application form is filled out. You should also personally sign the last paragraph with the decoding of your last name.

At the end of the questionnaire there are several points in which you also need to sign. In this case, we are talking about the consent of the Schengen visa applicant with the fact that in case of refusal Visa fees will not be returned to him. You will also need to agree with the need to process personal information and that you will have to take out a medical insurance policy covering the entire territory of the trip and its entire duration.

In the finished Schengen visa, the data that were indicated in the application form are affixed

Filling out the form online

In most cases, the Schengen visa application form is filled out manually in paper form and submitted along with the necessary package of documents. But some countries, in particular Estonia, Lithuania and Latvia, require you to fill out a visa application form online. On the one hand, this is easier to do, on the other hand, on the contrary, it is more difficult.

The rules for entering information remain the same. You must enter all the data yourself and send the questionnaire to the diplomatic mission. The system checks if all items are filled. If any field is left blank, the application will not be accepted. Online filling involves saving the questionnaire on the site under an individual identification number. Therefore, you can enter information in several stages, which is convenient if you need to clarify any data. Particular attention should be paid to the beginning of the electronic questionnaire: there you need to select from the list the diplomatic mission where documents will be submitted. There is no such item in the paper version of the questionnaire. If the consulate is indicated incorrectly, admission will be denied.

After filling out the questionnaire, it is printed, signed and submitted in this version. Additionally, you do not need to add anything by hand.

The application form for a Schengen visa can also be filled out online, if allowed by the diplomatic mission of the country to which the visa is requested

Photo Requirements

Together with the printed and completed application form for a Schengen visa, two visa photographs are also submitted. They should not be glued to the questionnaire, it is enough to use paper clips. Photos are signed on the reverse side. The applicant's last name must be written in English language, in legible handwriting, in the transliteration that is in the foreign passport and, accordingly, the visa application form. It is advisable to do this with a pencil, especially without pressing on the back of the photo.

The photo on the questionnaire must comply with the requirements of ICAO (International Civil Aviation Organization). It has dimensions of 3.5x4.5 cm, this is a passport format. The picture is taken on high-quality and high-contrast photo paper, printed on industrial equipment for photo printing. The image of the applicant occupies at least 70% of the total area on the visa photo. In no case should the color of his clothes merge with the background of the picture, which can be white, light blue or light beige. It is forbidden to be photographed in glasses with tinted lenses and in headdresses (except for applicants who wear it for religious reasons - women of the Islamic faith, Jews can serve as an example). You can take a picture with ordinary glasses, but in this case, the passport must contain the same photo of its owner. It is also desirable that the frame of the glasses is as narrow as possible and does not cover a significant part of the eyes. When photographing, it is necessary to be full-face, with a direct look, without a smile, with a hairstyle that does not cover the forehead and face.

Photos for a Schengen visa must comply with ICAO requirements

Often, even if the visa application form is filled out correctly, it is rejected due to an incorrect visa photo. If the applicant's appearance has changed significantly since the photograph was taken, new photographs will need to be taken. The visa photograph itself should not be older than six months.

To correctly fill out the application form for a Schengen visa, you must immediately, even before the start of this procedure, familiarize yourself with detailed instructions by filling. This will help to protect yourself from possible mistakes and significantly speed up the process of obtaining a visa permit to the Schengen countries.

The correct completion of the Schengen visa application form is crucial for a positive result of the request. It is this document that is the most informative for the visa officer, intentional errors or omissions in which are a direct path to denial of entry into the Schengen area.

You should not assume that if you failed to get a visa in one of the states of the union, you will succeed in another. Often, a previous refusal leads to difficulties or repeated rejections in other embassies. Therefore, clarity and compliance with all requirements should be decisive when filling out this paper.

In paragraph 11, you should indicate the series and number of the passport of the Russian Federation. In the questionnaires of some countries, this item is sometimes marked as "Identification number" - it is not in the Russian passport, this field should be left empty.

Item 12 - the type of travel document for which a visa is requested. In most cases, "Foreign/Regular Passport" should be checked.

In paragraphs 13, 14, 15 and 16, you must specify the data foreign passport: series, number, date of issue, expiration date and by whom issued. The name of the authority that issued the passport must be transliterated into Latin, for example: FMS → FMS, FMS → UFMS, etc.

This paragraph should indicate actual address of residence. If the actual address of residence differs from the address of permanent registration (registration), the consular officer may require additional documents, confirming the residence of the applicant at the specified address. Such documents can be: a lease agreement or a certificate of employment.

The specified address should not be translated, only transliterated. For example, the option "LENINA STREET" will be incorrect, the correct one is "ULITSA LENINA".

Item 28 - the country to which the traveler is going, if the trip involves transit through the Schengen area, and the destination country is not included in the Schengen country.

Points 29 and 30 are the days of entry and exit (multiple trips assume the first date as the day of the first entry into the Schengen area, and the second date as the last day on which the exit is made).

Paragraph 31 - if there is a person who invited a citizen to the Schengen zone (relative), indicate his full name, location and contacts. In the absence of such, it is necessary to indicate the name, address, e-mail address and phone number of the hotel (hotel) in accordance with the reservation.

Important! A sample of filling out the application form for a Schengen visa is always directly available at the stands of the visa centers.

Application for a Schengen visa for a child

At the end of the document, the applicant must put his signature. Minors do not have the right to sign until the age of 15. For them, the signature is put by one of the parents or a person who has legal grounds for that. A forged signature entails an unequivocal refusal of a visa. Parents or legal representatives of the child (guardians) must also fill in.

It is worth noting that paragraph 27 indicates whether the person handed over fingerprint data. For children under the age of 12, it is not required. And in paragraph 10 - the data of the parents (name, citizenship) and their registration, if different from the place of residence of the child.

In addition to signing in paragraph 37, parents should also sign on the 4th page of the questionnaire on familiarization with the rules of being in the Schengen area.

Despite the fact that some concessions for minors are provided for when issuing entry permits (for example, in relation to a photo), you should take full responsibility when filling out the questionnaire. Otherwise, the situation when the parents could leave, but the refusal was received for the child's trip cannot be avoided.

It is also worth remembering that the space indicated in the passport number is also considered a character, i.e. if they indicated the number "70 84839457", then the password would also contain a space - "70 [space] 809071990" .

Where to get the form

When applying for a Schengen visa on your own, you can take the application form for a Schengen visa directly at the consulate or visa center or print it from their official websites. You can do it even easier: fill out the questionnaire on our website, attach it to the application, and we will assess your chances of obtaining one or another permit, competently fill out the questionnaire based on your answers in the questionnaire and provide full assistance in the application process. With over 20 years of experience in processing Schengen visas, we can guarantee that any European consulate will make a positive decision on your visa issue.

Russian or English?

The application form must be completed in the state language of the country to the embassy of which it will be submitted, or in English. But there are exceptions. For example, the representations of Germany, Poland and Estonia are quite willing to accept applications in Russian.

Taking on the formation of a petition makes sense only if you speak English at a high level. In the presence of the slightest gaps in knowledge, it is wiser to turn to specialists. They will help you avoid annoying rejections due to minor errors in the application.

Where should the applicant apply?

Once a visa has been opened, its holder can travel throughout the entire Schengen area, since its conditions are the same in all entering countries.

But there are certain rules and regulations that relate to which embassy or consulate a candidate must apply for a Schengen visa.

If a person travels to one of the Schengen countries, he must apply to the embassy or consulate of the respective country.

Given that a person plans to travel to two or more Schengen states, it is highly recommended to apply for a visa at the embassy of the embassy where he will be staying for the most number of days.

If there is no main destination, but simply a visit to several countries, the candidate must apply for a visa at the consulate of the first one he enters.

Can I apply for a Schengen visa in a country where the applicant is not a resident?

Only permanent residents can apply for a Schengen visa outside their home country. For example, with a US green card, you can apply, but not with a tourist visa.

Primary requirements

All countries of the Schengen area in 2019 use a standard questionnaire form. Wherever you turn, you will be offered a document that requires 37 points to be filled out. The only difference will be the language and country name at the beginning of the questionnaire.

When starting to fill out the application form for a Schengen visa, the applicant must be prepared to honestly and carefully approach the indication of data.

Before applying for a document, you should decide on the type of visa, collect the necessary package of documents, familiarize yourself with the requirements for insurance, and obtain documents that can confirm the purpose of the visit and financial security.

This will allow you to carefully prepare for the execution of the document and increase the chances of satisfying the request.

This is how the Schengen application form looks like

In most cases, it is possible to fill in by hand, that is, with a good purple ballpoint pen. Filling out on a typewriter is welcome. Some countries do not recognize any other way to fill out other than online. This:

  • Estonia;
  • Norway;
  • Switzerland;
  • Slovakia;
  • Liechtenstein, etc.

It should be expected that in the near future the list will expand, as online filling can significantly speed up the process of reviewing documents, which suits not only representatives of foreign missions, but also applicants.

There are also requirements regarding the filling language. The questionnaire must be filled out in Latin letters, and the profession must be indicated in English or the national language of the country. The name and surname should be written exactly the same as it is written in the passport.

As for the dates, it is worth writing exactly the same as indicated in the paragraph of the questionnaire. If year-month-date is written, then the date is written only in this order and only through a dash.

Place and country of birth must be indicated as in the passport. If a person was born during the Soviet era, then "USSR" must be indicated. The same situation with renamed cities: if in the passport instead of Nizhny Novgorod stands the city of Gorky, then indicate it.

The questionnaire is on four pages. Some countries require that printouts be made on separate sheets, while others, on the contrary, require printing on both sides. This nuance can be clarified on the website of a particular Consulate or Embassy.

Standard set of documents that will be needed to fill out the questionnaire

Whatever country you are going to, you need to prepare, you will need some part of them, not only for delivery to Visa Center, but also as a model for filling in many columns. To fill out the questionnaire, a foreigner will need to provide the following information:

Having prepared Required documents, you can quickly enter data into the questionnaire. To avoid mistakes, do not forget to refer to the example of filling out the application.

Columns of the questionnaire and features of their filling

After collecting the documents, you can start filling out the questionnaire. With both paper and online options, you have the ability to rework the questionnaire and make changes to it. In online options, you can save and then return to filling from where you left off.

Questionnaire Questions

By filling out the questionnaire, the applicant undertakes to correctly answer the following questions:

The applicant notes that he is aware that in case of refusal, the money paid for the visa fee will not be returned to him.

An example of a visa fee payment receipt

The traveler also confirms his awareness of the need for medical insurance.

It is equally important to confirm your consent to the processing of personal data. It is noteworthy that the data can be disclosed at the request of the secret services of the Schengen countries. This is necessary for the timely disclosure of offenses.

At the same time, the person is informed that he has the right to protest if he discovers that his personal data has been processed illegally or incorrectly. The person also confirms that he is aware of the responsibility that he bears for providing false data.

Step by step instructions with specific examples

The questionnaire begins with personal data.

To fill in points 1 and 3, you need to take your passport as an example and transfer your last name and first name letter by letter. Item 2 is filled in only by those who now use not their first surname.

You need to enter the previous names separated by commas. Item 5 asks for the city where you were born. Even if it is not indicated in your passport, you still need to write the name. If you were born before 1991 and your locality at that time it was called differently, in most cases it is required to write the old name (it is advisable to clarify this issue or look at the sample).

This also applies to point 6, where the country in which you were born is requested, then if it was Soviet Union, in most cases you need to write USSR or SOVIET UNION. In paragraph 7, you need to indicate real citizenship (everything is clear with this) and, in some cases, citizenship at birth. Here again, USSR is written.

There is no need to explain how the gender and marital status fields are filled in. These graphs do not need examples. You actually fill them in.

Please note that column 10 was created only for minors. If you are filling out the questionnaire for an adult, you should skip it. In some cases, a dash or “does not apply” is put. In the questionnaires of minor citizens, data on parents or guardians are entered here:

  • Names, address and nationality of persons having parental rights, or details of the legal guardian.
  • Signature of the person with parental rights. Relationships should be explained in parentheses (father, mother, guardian).

The parents of a minor applicant must provide a written (and signed) consent to travel with a parent or trustee. You can make an entry in the corresponding column of the application form.

If you are a citizen of the Russian Federation, there is no identification number in your passport, therefore point 11 has nothing to do with you. Citizens of other countries must open their sample (passport) on the desired page and rewrite the numbers from there.

Sample of filling out the second sheet of the Schengen visa application form

Be careful when filling out boxes 22 and 23. Some countries require that both the country of destination and the country of first entry be their country. When filling out the 24th paragraph, think about what kind of visa you need.

If you are a tourist with a ticket and booked tickets and a hotel room, check the box "once". This is important because if you intend to apply for a double or multiple entry visa, you will need to justify and document this decision.

In Item 26 you need to write about the Schengen visas that you received during the last three years. If there were none, put "NO".

Paragraph 27 deals with the collection of biometric data, which has become a mandatory measure for many countries. If you have booked tickets, take them as a sample and write out the dates in paragraphs 29 and 30. For an example of writing a date, see the sample of filling out a questionnaire for a specific country.

Paragraphs 31-33 are devoted to the inviting party, fill in 34 and 35 only if you have relatives in the EU. In paragraph 36, you need to write where the application form is submitted with the rest of the documents, in paragraph 37, a signature is put. The last two paragraphs are duplicated at the very end.
